A personalized one-on-one Wellness Coach guides a member through an individualized plan of care related to specific needs. A Wellness Coach can help members establish personal health goals, provide members with educational information and counsel members on moving toward positive behavioral change.
If a member faces challenges with excess weight, high blood pressure, high cholesterol, stress or smoking -- often identified by online assessments or annual physicals -- the member qualifies for a personal Wellness Coach. The Wellness Coach collaborates one-on-one with the member to discover interest areas, and any recurrent issues that need to be addressed or challenged in a way that creates positive change. The Wellness Coach will examine a member's thinking and belief systems from a broad, healthy perspective and help the member work toward achieving specific, measurable goals.
HealthSmart's Wellness Coaches are professionals with a degree in Psychology, Health Science, Social Work or Nursing. In addition, they have a background in one or more of the following:
- Certified Personal Trainer
- Licensed Practical Nurse or Registered Nurse
- Marriage and Family Therapist
- Licensed Clinical Social Worker
- Marriage, Child and Family Counselor
