A Provider is another term for a provider of healthcare services. Providers include doctors, hospitals, laboratories, clinics, and other persons or organizations that provide healthcare. If you have a favorite doctor, hospital or other provider that does not participate in your HealthSmart Network, please complete a request form. We will gladly contact your provider about joining a HealthSmart network and enjoying all the benefits of being a HealthSmart preferred provider! In addition, we will contact the providers you nominate and send them the necessary applications.

Please note that we thoroughly investigate a provider’s history before inviting the person or organization to join a HealthSmart network. Our personnel vigorously verifies the provider’s credentials and background before inclusion into one of our networks.
